2025 SAFP Midyear Vendor Registration Form
Please fill out the following form to the best of your ability. Should you have any questions or concerns, please email the SAFP Webmaster at safpwebmaster@gmail.com.
Bronze Package (15 packages available):
- 2 Vendor Badges
- 8X10 Booth w/ 6 ft Table & 2 Chairs
- Two Daily Breaks with attendees
- List of Attendees & Contact Information
- Company Name in Program

Silver Package (10 packages available)
- Everything in Bronze plus preferred booth placement
- 4 Total Vendor Badges
- Invite to Sunday Icebreaker Social
- 1 extra space in Hallway or Vendor area for sign or promo material (vendor provides)

Gold Package (10 packages available):
- Everything in Silver plus best booth placement
- 6 Total Vendor Badges
- Invite to the SAFP Awards Dinner
- 2 extra spaces in Hallway for sign or promo material (vendor provides)
- Invite to lunch with Key Air Force Pharmacy Leadership
- Logo on Souvenir T-Shirt
Additional vendor badges above tier allotment are $500/each. If no additional members are attending, please leave this space blank.
